MRI events and near misses are inevitable even in the most safety conscience departments. While these events need to be reported, it is also important to learn from them and make appropriate changes. Creating an effective process to analyze such events and identify the root cause can help strengthen a department's safety practices and policies. This presentation will walk you through a journey our multi-hospital network went through to create such a process. We were able to establish a review method that can be conducted in a confidential manner, that focuses on underlying contributing factors, and create opportunities to create better uniformity across all network MRI departments. We will also share how this approach helped improve employee engagement in MRI safety, strengthen our protocols, and decrease the number of preventable events.
